- A Cancellation Fee of $50 or more may be assessed for any cancelled home testing kits. The shipping envelope for Home Testing kits must remain unopened and returned to us. Please call for approval on returns.
- A Cancellation Fee of $75 or more will be assessed for in office appointments. There will be no additional charge for rescheduling appointments.
- Cancellations can only be completed before 90 days from the order date. We can reschedule appointments up to 1 year later.
- A $6 charge will be added to the total cost of testing if shipping an extra kit to another address.
- All tests must be completed within 1 year of the order date.
