A Cancellation Fee of $50 or more may be assessed for any cancelled home testing kits. The shipping envelope for Home Testing kits must remain unopened and returned to us. Please call for approval on returns.

A Cancellation Fee of $75 or more will be assessed for in office appointments. There will be no additional charge for rescheduling appointments.

Cancellations can only be completed before 90 days from the order date. We can reschedule appointments up to 1 year later.

A $6 charge will be added to the total cost of testing if shipping an extra kit to another address.
All tests must be completed within 1 year of the order date.
